How do you tell the difference between hair loss and breakage?
The simplest and most reliable method requires nothing more than looking at the strands you find in the brush, shower, or on your pillow.
Hair loss starts at the scalp and often includes strands with a white bulb at the root, while hair breakage occurs along the hair shaft and produces shorter, uneven pieces with no bulb.
A full-length strand with a small white or pale club root at one end has completed the telogen resting phase and shed from the follicle naturally. This is what 50-100 hairs per day look like — normal exogen, the fourth phase of the hair growth cycle. An increase in these hairs indicates the follicle is sending more hairs into telogen than usual — which points toward an internal driver: hormonal, nutritional, inflammatory, or stress-related. This is the category that the last three months of this series has been addressing.
Broken strands do not have a white bulb attached. They are shorter, uneven, and often found in variable lengths — from a few centimetres to mid-shaft. Breakage hairs may have jagged, frayed, or split ends. They are found more often on pillowcases, in hairbrushes, and around the crown or mid-lengths rather than at the front hairline. The follicle is producing hair normally — the strand is simply not surviving long enough to reach full length before snapping. This is a shaft integrity problem, not a follicle problem.
Hair loss and breakage frequently occur simultaneously in the same person. Someone with AGA and regular heat styling may have both follicle-origin miniaturisation AND shaft breakage from thermal protein damage. Someone with telogen effluvium from iron deficiency may also have breakage from the same nutritional deficiency that weakens both follicle function and shaft integrity. If it has a small white bulb, it's a natural shed. If it's short and has jagged ends, it's breakage. Both can be happening at once — meaning there is a problem with both the follicle and the hair shaft. This is why looking carefully at the mix of hairs in your brush matters.
What causes hair breakage?
Hair breakage happens when the hair shaft loses its strength. Common causes include excessive heat styling, chemical treatments, lack of moisture, mechanical stress, and environmental damage.
Can hair breakage cause hair loss?
Breakage does not directly cause follicle-origin hair loss — the two are mechanically distinct. However, they can create conditions that amplify each other. Traction from tight hairstyles that causes breakage can also, over time, cause traction alopecia — permanent follicle damage at the hairline, which we covered in the June article. Chemical processing that produces shaft breakage can also sensitise the scalp surface and contribute to the inflammatory environment that feeds PIILIF. Hard water mineral deposits that weaken the shaft also disrupt scalp pH and microbiome.
The distinction between breakage and follicle-origin loss is real and clinically important. But the lifestyle factors that cause breakage often also stress the scalp environment in ways that are relevant to follicle health. They are separate mechanisms that frequently share common causes.

Yes. Iron deficiency and protein deficiency both affect follicle cycling (causing telogen effluvium) AND shaft quality (weakening the keratin structure that gives the shaft its tensile strength). Someone with both low ferritin and inadequate cystine/methionine intake may have follicle-origin shedding AND shaft breakage simultaneously — the white bulb test will show both types in the brush.

Check your brush, your pillow, and the shower drain. Separate a few strands and look at them in good light.
